Phoenix Children’s honored for commitment to patient safety by national hospital network

Phoenix Children’s has been named December Hospital of the Month by Children’s Hospitals’ Solutions for Patient Safety (SPS), a network dedicated to eliminating serious harm in pediatric hospitals. The recognition highlights Phoenix Children’s ongoing efforts to improve safety for both patients and staff.

SPS is a collaborative network that includes more than 140 children’s hospitals across the United States and Canada. The group promotes transparent sharing of data, best practices, and successful strategies among its members. This approach, described as “all teach, all learn,” aims to develop prevention standards and foster a culture focused on reliability and partnership with patient families.

“At Phoenix Children’s, safety is paramount. We follow a ‘Safety II’ framework, empowering our teams to model safe behaviors and recognize the everyday actions that protect patients and improve outcomes,” said Kelly Kelleher, MD, chief quality and safety officer and associate chief medical officer. “Our involvement with SPS guides our ongoing efforts to enhance safety, quality and the overall care we provide.”

One key element of SPS is error prevention. At Phoenix Children’s, all staff participate in Error Prevention training designed to set clear expectations around safety and reduce human error. Other measures include regular safety huddles and simulation exercises aimed at achieving zero harm.

Earlier this year, Phoenix Children’s received ISO 9001:2015 certification. This international standard recognizes organizations that demonstrate consistent delivery of high-quality care through clear processes and shared goals.
